چکیده

Osteochondral lesions are drilled with the aim of stimulating the growth of new cartilage to cover the defect (Pnidie 1959). This may be performed at arthrotomy or anthroscopically, but arthroscopic drilling is far from standard practice. Performed percutaneously, arthroscopic drilling by Kirschner wire can be hindered by difficulty with accurate placement, skidding off sclerotic bone and entanglement with soft tissues. A needlescope sheath (Guhl 1982), an instrument cannula or the operating arthroscope’s instrument channel

Arthroscopic management of talar dome lesions using a transmalleolar approach.

Surgical treatment of posteromedial talar dome lesions is frequently necessary for Berndt and Harty grade IV osteochondral defects and nondisplaced osteochondral fragments resistant to conservative modalities. When operative intervention is indicated, the approach and management can be complicated by the location and extent of the injury. The operative technique we advocate allows direct exposu...

Arthroscopic Revision Repair of Osteochondral Lesion of Talus Using Microfracture Drill and Bioengineered Dehydrated, Micronized Allogenic Cartilage Extracellular Matrix

Osteochondral lesions (OCD) of the talus are a common cause of chronic ankle pain and dysfunction, especially with history of injury and trauma to the ankle [1-3]. Lesions that do not respond to non-operative treatment may require surgical intervention arthroscopically or as an open procedure. Revision repair of osteochondral lesions historically has been performed as an open procedure with inc...

Diagnosis of Knee Osteochondral Lesions With Ultrasound Imaging.

Evaluation of articular cartilage and subchondral bone is essential in the diagnosis of joint diseases and injuries. Interobserver and intraobserver reproducibilities of arthroscopic grading are only poor to moderate. Thus, for quantitative and objective evaluation of cartilage and subchondral bone, ultrasound arthroscopy (UA) has been introduced to clarify this dilemma. Assessment of the clini...
